There is generally one employee responsible for processing all new petitions, overseeing 210 reviews, and docketing all entries for these cases.   All Adoption cases are impounded by the court.

CJ-P-87 Petition for Adoption To Petition the Court for Adoption.  Must be printed on bonded paper, acid free paper.
CJ-P-96 Affidavit of Petitioner for Adoption  
OCAJ-1 TCR IV Affidavit Disclosing Care and Custody Procedure Must be filed with all petitions/complaints wherein custody is an issue
  Page 2 - Affidavit Disclosing Care and Custody - Confidential Address Form Must be filed with the Affidavit Disclosing Care and Custody if your address is to be kept confidential
OAS: 300 Federal & Central Registers of Missing Children Search Request Form must be printed on bonded, acid free paper. 
CP-2 Request for Record Information Must be completed for EACH petitioner.
